Referral process:
1. Conduct a consultation with the patient & conclude on possible POM requirement.
2a. Discuss & approve referral/ consultation cost with the patient & call HLC to discuss clinical issue. 2b. Connect patient to have consultation with HLC.
3. Fill and submit the referral form below to HLC.
4. Private Rx will be sent to referral Pharmacy for dispensing.
5. Take consultation charge from patient on behalf of HLC
